Every doctor has considered it, but a few have even developed their own rules for determining when it’s time for a patient to go.

To be sure, as the colleges of physicians and surgeons will sternly tell you, doctors are only allowed to accept unsolicited gifts from patients that are ‘modest tokens of appreciation.’
