Why I stopped blaming patients for no-shows

For years, I took it pretty personally when someone didn’t show up. You know the drill: the 10:30 a.m. time comes, nobody shows, you get annoyed and a bit smug. I mean, we did our part holding the time and all, but they just weren’t there. So you jot it in the chart, the front desk gives a sympathetic head shake, and everyone thinks it’s the patient’s fault.But I changed my view on this. It wasn’t because of some fancy article or seminar; it came from actually considering the obstacles we put in patients’ paths. We expect them to keep appointments, but we may have quietly made that tough.Listen, I’m going to say something that will probably ruffle feathers: most no-shows don’t stem from a patient’s character flaw. They often come from the hassles we’ve built into our own processes. If that’s true, then high no-show rates aren’t really about patient behaviour at all. They point to operational issues we’re putting on patients.Look at what we actually ask of peopleConsider a fairly ordinary follow-up appointment. A patient gets a card or a voicemail weeks in advance. Sometimes there’s a reminder call the day before, but the patient doesn’t always answer because the number is unrecognized. The appointment is at 2 p.m. on a Tuesday, meaning leaving work and arranging childcare. Then, there’s the waiting room that’s running 40 minutes behind schedule. If they need to reschedule, patients call during clinic hours, wait on hold and often can’t get through. And if they have a question that might’ve spared them an unnecessary visit, there’s no easy way to ask it.Imagine that same person also doesn’t have a job that offers paid time off, speaks limited English or has a chronic condition, making some days unpredictable. Our system works wonderfully for an organized, flexible patient who speaks English and has a lenient employer. Yet, we’re shocked when others struggle.When I mapped our clinic’s no-show patterns, I noticed those missing appointments faced specific issues. It wasn’t random; it clustered around predictable friction points. This isn’t coincidental. Many of these barriers are built into the way our systems currently operate. The ‘blame’ reflex costs us more than the empty slotThis is the part that bothers me as a doctor, not just as someone running a business. If we label no-shows as patient irresponsibility, we halt our investigation there. It’s already decided: the patient simply didn’t care enough to show up. Because of this mindset, we fail to question why certain groups are missing appointments and we overlook key clues in all the day-to-day chaos.Patients who miss daytime slots might be the ones whose diabetes is getting worse. The person constantly rescheduling their colonoscopy could be the very individual panicking over potential bad news. Now, here’s the thing: no-show rates aren’t random; they align with the very patients we’re most concerned about. Calling it laziness or forgetfulness means missing crucial opportunities to step in and help.Once we stopped asking who was to blame and started asking where patients were struggling, the solutions became surprisingly obvious.What changed when I owned the frictionI changed my mindset from “How do we punish no-shows?” to “What makes keeping appointments tough?” The second approach actually helped me see improvements.Many changes were pretty basic. Like sending appointment reminders through methods that patients prefer, not what we find easy. Also, making rescheduling a breeze, taking just 20 seconds and avoiding a call to a person. Plus, setting up quick question options so folks don’t have useless visits that might look like missed appointments.These steps aren’t earth-shattering. The tricky part wasn’t implementing fixes; it was acknowledging that our clinic might be part of the problem. We had to honestly examine why we were running behind schedule and address it. After all, chronically late services teach patients that their time isn’t valuable. Who shows up when they think the system treats them like that?So, what really mattered was letting go of the idea that the issue lies solely with patients. Working on ourselves was just as important.Who owns clinic friction?I’ll be upfront about my bias here. I’ve been working with a platform company—HiClinic.ca— that aims to decrease clinic friction, so take what I say, keeping in mind that I benefit professionally and commercially when clinics smooth things out. That said, I truly believe in my argument, which I’ve held since before I ever got involved with this tech.My point is simple: clinic issues are the clinic’s responsibility. Patients have duties too, but ultimately, if there’s a problem, we created it, and we’re the ones who can fix it. Patients can’t alter our processes; only we can adjust our systems, scheduling, and waiting times.It’s easier to blame patients. There’s no cost to us, and it demands nothing from us. Yet, this attitude just lets us off the hook and lets down the patients who are counting on us to do better. I quit pointing fingers at patients for no-shows after realizing that empty chairs often reflected more on my clinic than on them. The easiest thing I could have done was keep blaming patients. The harder and more useful question was whether my clinic had earned their attendance. That’s the question I ask now.
