When the roads forget them

There are days that are heavier than others. Days when our words change the pace of someone’s life. Those are the days that stay with us after we leave the room, as we wipe the wetness on our sleeve and glance at the quiet corridors ahead of us. One of those days is when we tell a patient that they can no longer drive.It is not about the license, or traffic laws or how much time it would take for them to safely reach a hospital. It is about something deeper than that. For many of them, a car is not just a car. It is freedom, independence, choice; it is the small reassurance that they can still decide when to leave and where to go, and in a quiet moment they are still in control of their own destiny. So when we tell them that it is all over, it is like life drawing a line in the sand: “You are no longer who you were, who you think you are.” Patients tell us that they felt something in them broken, something stolen. They quietly retreat to their chair, defeated, as though something inside them was snatched by an invisible pair of hands, to which they can never show. That private loss, the kind that few will ever witness, is what sinks in their hearts.The reaction is never simple. Some sit there, in stunned silence, not knowing what to say. Some cry, many are angry not because they do not know, but because they do. Angry because all of a sudden, not only are they suspended from the wheel, but the harsh glare of life is upon them, unveiling before their very eyes what they have been desperately trying not to see. And sometimes they are angry at the physician in front of them, the messenger of loss, the dispenser of power, the one who can suddenly, with authority, say “enough.”Loneliness, then follows. The world becomes smaller when they can no longer drive themselves to it. The same trip that used to take them five minutes now requires someone else’s time and goodwill, so patients quickly confess that they feel like a burden, even though we tell them they are not. To ask for a ride is a heavy request: each time they dial a number, a cloud of guilt follows.Some of them adjust to it. Many, with the loving and dutiful support of their family, find ways to cope, lean on others, walk, wait, change. But even when they tell us that they are fine with it, we still know the silent pangs it carries. The ache that lingers as they sit in the passenger seat and watch someone else grasp the wheel, as if to say “you are not trusted to hold on anymore.” A parent who once drove their family becomes dependent. A partner who once was driven places suddenly finds themselves driving instead, caretaker at the wheel. It is a practical change, of course it is, but it is also an emotional wound that takes time to heal, if ever.As physicians, we see the rationale behind it. We witness the private heartache as it unfolds. We watch their eyes lower and their voices tremble as they try to talk about it, about “what will I do” and “who will drive me,” and their quiet despair as the decision is made. It stays with us after we leave the room. Because reporting them and then taking away a license is not about a privilege: it is about taking away a part of who they are.The hardest part is not the suspension only.
