The thoughts we tell patients to stop

Recently, I was attending a course on the general topic of recovery when the concept of thought stopping was introduced.It was presented simply, almost reassuringly, as a practical tool for behavioural change: when a distressing or unhelpful thought arises, interrupt it, replace it, don’t let it take hold. The logic was intuitive, even familiar, the kind of advice that feels helpful in its clarity.And yet, as I listened, I found myself unsettled—not because the idea was new, but because it wasn’t. It echoed something I hear often, both in the language patients bring into the room and, at times, in the guidance we offer them. And it stood in quiet contrast to much of the work I do, which asks patients not to control their thoughts more tightly, but to relate to them differently.We see, every day, how much suffering is created not only by the thoughts themselves, but by the effort to contain them. How quickly the mind can become something to manage, to discipline, to override. And yet here, in a space meant to support recovery, the message was clear: stop the thought, push it away, regain control.I began to reflect on the ways this erroneous idea shows up in my clinic, often without being named as such, but shaping the way patients understand their own minds.One patient describes how thoughts about food have come to structure her entire day, not in a casual or passing way, but with a kind of persistence that feels inescapable. She wakes up thinking about what she will eat, spends the day trying not to eat it and goes to bed replaying every decision she made.“If I could just stop thinking about it,” she tells me, “everything else would follow.”Another patient speaks about pain—low, constant and increasingly difficult to ignore. What began as a physical sensation has become something she tracks almost automatically, her attention returning to it throughout the day, measuring its presence, anticipating its return.“I keep telling myself not to focus on it,” she says, “but the more I try, the more it’s there.”A third, navigating the hormonal shifts of perimenopause, describes the way her mood has begun to move in unfamiliar ways, accompanied by a growing awareness of the thoughts that seem to precede it.“I can feel it starting,” she says. “I try to talk myself out of it, but once it’s there, I can’t stop it.”Across these conversations, the details differ, but the underlying belief remains remarkably consistent: if the thought can be controlled, the problem will resolve.It is not a belief patients arrive at in isolation. It reflects something deeper in the way we understand both illness and recovery, a tendency to equate improvement with control and to extend that expectation to the mind itself. For a time, this idea was formalized into approaches like thought stopping, which offered a structured way to interrupt unwanted mental content. But what has become increasingly clear, both in the literature and in clinical practice, is that the mind does not respond well to force.The more we try to suppress a thought, the more persistently it returns. Social psychologist Daniel Wegner described this paradox decades ago, noting that in order to avoid a thought, the brain must continue to monitor for its presence, asking, often outside of awareness, whether it is still there. In doing so, it keeps the thought active.This is not an abstract phenomenon; it is what our patients describe. The effort to stop thinking about food makes the thought more central. The attempt not to notice pain sharpens awareness of it. The effort to control mood increases vigilance around its fluctuations. What begins as an attempt to regain control can become part of what maintains the experience.And when it does not work—as it so often doesn’t—it leaves patients not only with the original distress, but with a growing sense that they should have been able to manage it differently, that the failure lies not in the strategy, but in themselves.What is striking, over time, is how often the shift begins not with the introduction of a new technique, but with the gradual recognition that control is not achieving what it promised. Patients begin, sometimes tentatively, to experiment with something else. The patient who has spent months trying to eliminate thoughts about food begins instead to notice them, to observe when they arise and what happens if she does not immediately respond. The patient with chronic pain allows the sensation to be present without tracking it as closely. The patient navigating mood changes recognizes that the thought is not a directive, and that the feeling will move, whether she fights it or not.These are subtle shifts, but they reflect a different kind of learning—one that does not depend on eliminating the thought, but on loosening its hold.This becomes even more evident in patients whose diagnoses mark a change that cannot be reversed, where the instinct to suppress thought takes on a different tone. The message, often well-intentioned, is to stay positive, not to dwell, not to go too deeply into what has been lost. But what cannot be undone does not respond to avoidance, and the more tightly these thoughts are held back, the more insistently they return, shaping mood, attention, and even physical experience.In these moments, what patients often need is not redirection, but permission—to think about it, to feel it, to allow the reality of it to take shape in a way that can eventually be integrated.As physicians, we are not only responsible for the treatments we prescribe, but for the frameworks we reinforce. When we encourage patients, directly or indirectly, to silence their thoughts, we risk reinforcing a form of internal resistance that can deepen distress rather than relieve it. Thought stopping is not simply ineffective; it can be subtly harmful in the way it teaches patients to relate to their own minds.Part of our role, then, is to recognize when commonly accepted advice no longer serves the people in front of us, and to be willing to offer something different, even when it feels less intuitive or less concrete.Wellness does not begin with the elimination of unwanted thoughts. It begins with a shift in how those thoughts are met.
