The park as preventive medicine

Prevention is the best medicine, and also the least practised. One of the strongest predictors of decline in older adults is not a lab value, but the gradual loss of mobility, strength and balance. As these erode, confidence diminishes. The result is as predictable as it is tragic: more pain, more falls, more dependence and greater healthcare utilization.We already know that regular physical activity reduces falls, preserves independence, improves mood and delays functional decline. A healthcare system that truly prioritizes prevention must prioritize mobility—not just in therapy sessions, but in daily life. Safe, accessible movement should not be the exception, but the norm. Walking, gradual strengthening, stretching and repetition are not “extras;” they are part of the foundation of dignity in aging.Public physiotherapy coverage for seniors is one important effort toward this goal. For many patients, physiotherapy is vital and irreplaceable. Post-operative rehabilitation, stroke recovery, falls rehabilitation, severe osteoarthritis and neurological or complex mobility disorders require structured physical rehabilitation. In these cases, physiotherapy can restore function, reduce disability and preserve independence.In day-to-day primary care, however, many physiotherapy referrals are requested for problems that are minor, self-limiting or can be effectively addressed through regular movement and gradual conditioning. This does not occur because patients are irresponsible or unmotivated. Often, pain erodes confidence, fear of falling discourages activity, loneliness diminishes motivation and depression quietly robs initiative. When a service is made free and accessible, it can unintentionally become the reflexive option—even when walking, movement and gradual strengthening may have been the most appropriate prescription.Many physiotherapists themselves emphasize that long-term recovery depends less on passive treatment and more on sustainable daily movement and conditioning. Yet when I recommend something as basic as going for a walk outside, weather permitting, starting slowly and gradually increasing distance and activity, I sometimes encounter resistance. The structure of a formal physiotherapy program often feels safer, more official and easier to commit to. It is scheduled, time-limited and publicly funded.The result, however, is frequent referrals, heavy system cost and sometimes the medicalization of problems that could improve through consistent low-intensity activity and community-based rehabilitation. When this occurs, public healthcare dollars may end up supporting passive consumption instead of enabling prevention.Some will suggest the solution is straightforward: physicians should refer patients only when strong clinical indications exist. Theoretically, this is true. In practice, however, those who say this may underestimate what the doctor–patient relationship can become when a request is denied, even when the denial is clinically appropriate. A denial can feel like a rejection. Trust can erode, and a medical interaction can slowly become a negotiation rather than a partnership.During a trip to Turkey, I observed something that deserves serious attention: exercise and rehabilitation devices in public spaces, available to anyone for physical training and basic rehabilitation-style activity. These stations were not commercial gym equipment. They were designed around practical movement, mobility, balance and gradual strengthening.I tried them myself. We would take a walk or a biking trip every day, stop at these stations for half an hour and move through the exercises one by one. I found benefit in the simplest and most meaningful way possible: through routine, movement, consistency and gradual improvement.What impressed me most was how naturally these stations addressed the very issues many older adults struggle with every day: balance, endurance, joint mobility, gait stability and lower-limb strength. They reduced the barrier to exercise. Many people avoid gyms because of cost, discomfort, intimidation, transportation or the planning involved. A public rehabilitation station removes much of that barrier. It becomes part of ordinary life: a walk, a short routine, and gradual conditioning. In rehabilitation, consistency often matters more than intensity.They also address another underestimated contributor to decline: isolation. Older adults deteriorate not only from arthritis or weakness, but because many eventually stop leaving home. A park-based rehabilitation station can create a social environment where seniors meet other seniors, exchange encouragement, share routines, and remain connected to community life. That is not only physical rehabilitation, but also quiet mental-health intervention and prevention of isolation.A reasonable question is whether these stations are safe for everyone, especially frail older adults. This is a valid concern and one that must be addressed carefully. It should not be an argument against the idea, but an argument for proper design. Equipment can be built using safety and accessibility standards, with clear instructions and warning labels. Communities can offer periodic guided sessions and physicians can counsel patients regarding suitability, red flags and gradual progression. Responsibility should be shared through safe design, proper guidance, and patient awareness.Canada’s climate is also very different from Turkey’s and this reality must be recognized. Yet this is not a barrier so much as a design challenge. Stations can be placed in areas with winter maintenance, in covered community parks, along cleared walking trails or adjacent to community centers where seasonal access remains possible.I believe it would be worthwhile to pilot similar publicly accessible exercise and rehabilitation stations in Canada. This would not replace physiotherapy but complement it as a broader public-health intervention. Such spaces could reduce unnecessary referrals, support mobility in aging, improve mental well-being, encourage social connection and ease some burden on the healthcare system.In a time of rising demand and constrained resources, it is prudent to invest not only in appointments, but also in environments—not only in funded clinics, but in funded community.
