Family medicine is one of the few specialties in medicine that everyone believes they understand.It is also one of the few specialties that almost everyone simultaneously underestimates.Ask the average person what a family physician does and you will usually get a simple answer. Words like prescriptions, runny noses, blood pressure and maybe an infection here or there. When discussed in policy circles, it is often reduced even further; distilled into a trendy phrase that has appeared repeatedly over the past few years: the family physician as a primary health provider.It makes us sound administrative, neutral and interchangeable.Family physicians aren’t defined by knowledge of a single organ. We are defined by things that are not easily defined.Most specialties enter the picture after the mystery is already partially solved or named. The patient arrives with a referral or an abnormal lab. Their problem has been placed neatly into a familiar box and the specialist is called upon to confirm and treat—rarely to define the diagnosis itself.Family medicine, on the other hand, often starts at the opposite end of the spectrum.The symptoms are nonspecific.The history is unclear.The tests come back normal.And you listen, and treat, and ask the patient if there is anything else worrying them, despite their insistence that there is not.This is when the patient returns to the family physician.Every lab has been checked. Every test has been performed. Yet here they sit in your office because they still don’t feel quite right.Their physical symptoms are compounded by anxiety, trauma and sometimes even personality difficulties that make it hard to separate one from the other.One doctor looks at their heart. Another looks at their kidneys. But no one steps back to look at the patient as a whole, assuming that someone else has already done so.Family physician. We accept the handoff.. . . the handoff that we have come to jokingly call dumping.But that term is wrong. Because when used in the right context, it is not dumping at all.It is an acknowledgement, however subtle, that this patient’s problem no longer fits into the neat boxes that modern medicine has confined itself to. That the books no longer have an answer, and it is now on us to figure out what comes next.So the mystery returns to us, and the patient is sent back to the place where medicine is least defined; back to the specialist of uncertainty.Back to family medicine.Family medicine is rarely glamorous. There are no operating rooms or prestigious procedures attached to it. What we do largely takes place in conversations, in careful history and examination, in patterns, clinical intuition and in building a relationship over time that allows us to see what the tests cannot reveal.Family medicine requires a tolerance for ambiguity that few other specialties are asked to carry. We allow patients to sit with us without forcing quick answers. We see them year after year, learning to recognize subtleties that others may miss. We connect symptoms that appear unrelated. We recognize when the body is speaking, but the true problem lies in the patient’s life beyond the clinic.From time to time, you will hear a newly graduated physician refer to family physicians, sometimes even in front of patients, as “just” a primary health provider.It reflects a misunderstanding of what family medicine truly is.You will also hear policymakers speak of family physicians as interchangeable with other providers, approaching the issue as though it were simply one of access. As though the work could be reduced to managing straightforward conditions, like treating a urinary infection.But treating a urinary infection is not the defining work of family medicine.Treating uncertainty is.When the history and physical do not match the textbook.When medical illness, mental health and life itself refuse to separate into clean categories.When each individual issue appears small, but together they quietly take over the patient’s life.That is family medicine.Sitting with the patient, working through it step by step, until a path forward begins to take shape.It is in these moments, when patients slowly begin to open up and share their stories, that you are reminded why you became a physician in the first place.And when medicine reaches the place where certainty fades and the pieces no longer fit easily together, that is where family medicine begins.The specialty of uncertainty.Dr. Jabir Jassam is a family physician in Ottawa.
