The hidden cost of targeted surgical funding

The conversation surrounding Ontario’s surgical wait times often focuses on a lack of funding. However, as a clinic manager overseeing a specialized otology practice in Ottawa, I am witnessing a more insidious crisis: the cannibalization of surgical care by “priority-funded” mandates. Our private clinic currently manages a backlog of 175 patients—individuals with chronic ear disease, eroding cholesteatomas and profound hearing loss. Despite our best efforts, these patients remain in a state of indefinite delay. The reason is a systemic “zero-sum game” in our operating rooms. When the Ontario Ministry of Health provides targeted, “ring-fenced” funding for high-profile initiatives—such as cochlear implants—that money often comes with mandated surgical priority. While these programs are vital, they do not exist in a vacuum. In a hospital system with fixed nursing hours and anesthetic coverage, every “protected"”block for a funded program is a block effectively robbed from the general surgical pool. This creates an ethical triage where favoured programs are prioritized over clinical acuity. A patient with a precarious ear condition is forced to wait because their surgery doesn’t carry the same “incentive” as a mandated, priority-funded condition. Furthermore, this funding model actively disincentivizes recruitment. We are facing the retirement of our specialized surgical practice in late 2027 with no successor in sight. Why would a young otologist move to a region where the majority of OR time is locked into specific carve-outs, leaving no room for a comprehensive, sustainable practice? If we truly want to address the surgical backlog, the government must stop funding procedures in silos.
