Building a pharmacy that doesn’t depend on me

There’s a kind of to-do list that doesn’t motivate you. Rather, it just stares back. I’ll open my laptop planning to do one small thing, and suddenly, I’m looking at payroll, followed by compliance reminders, bills, half-written SOPs, equipment logs and emails I’ve been meaning to answer. It’s a running list of things that feels less like tasks and more like quiet accusations.“You should’ve done this already,” you tell yourself. “You’re behind. This is on you.” Nothing is technically on fire. But my chest tightens anyway.If you own a pharmacy—especially a compounding pharmacy—you probably know this feeling. A lot of pharmacists have no interest in becoming pharmacy managers. I understand why because I used to be one of them.The math never quite added up. A modest pay bump in exchange for significantly more responsibility, regulatory exposure and personal liability. You’re accountable not only to the pharmacy, but also the college, corporate or ownership. Yet, you’re still expected to perform as a frontline pharmacist.It never felt worth it. For a long time, I was sure the only way I’d ever take on that role was if I owned the pharmacy myself. And then I did. What I didn’t fully appreciate at the time was how much of a toll the role actually takes on you, especially in a compounding pharmacy like mine. The responsibilities don’t just add up. They stack, and then they keep growing.You’re responsible for dispensing and clinical care, but also for compliance, training, documentation, audits, equipment maintenance, risk assessments, SOPs, quality control, staff competency, and staying ahead of ever-evolving standards. In the early stages, most of that lands on one person. However, in compounding, the technical work is rarely the hardest part. It’s the systems that need to exist so work can be done safely and consistently: the documentation that needs to be created, maintained, reviewed, and actually used, not just filed away for inspection day.That’s where things start to feel heavy. Because even when you have someone wearing the hat of a compounding supervisor, there’s still the question of how you stay assured everything is running the way it should, and what happens if they walk away. Especially when you realize that much of this work isn’t optional, and yet isn’t always visible or valued.This is probably why so many pharmacy owners feel tethered to their businesses. Not because they want control, but because so much responsibility lives in their heads. Stepping away doesn’t feel like rest, it feels like a risk.And when people say, “Just hire a manager,” it can feel disconnected from reality. Bringing someone into that role takes more than finding room in the budget for their salary. They need systems that work, clear expectations and a job they can actually do without ending up just as burned out as you are.Here’s something I’ve started to realize: It’s not that pharmacists don’t want leadership roles. It’s that many leadership roles haven’t been designed to be sustainable, especially in highly regulated environments like compounding.Lately, I’ve been asking a different question. Instead of “How do I find someone willing to take this on?” I’ve been asking, “What would make this role reasonable for someone else?”What systems would need to be in place? What decisions shouldn’t live in one person’s head? What documentation should guide, not overwhelm? Where is money better spent on prevention instead of cleanup?Those questions have changed how I think about management entirely. I don’t have all the answers. But I’m starting to see the patterns.I’m seeing how the right systems can turn what once felt like a terrifying role into a manageable one. How designing intentionally for continuity, instead of just survival, changes everything. And how avoiding certain early money pitfalls actually creates space for better leadership later on.That’s the work I’m in right now. This blog is where I’ll share what I’m building as I go: the systems I’m putting in place to support business continuity and the decisions that make it possible for the pharmacy to function safely and smoothly, even when I’m not carrying every detail myself.If you’re on this journey—or even just thinking about stepping into it—you’re in the right place.
